Care is based on agency policies and procedures through the competent application of the nursing process.The LPN is responsible to the Clinical Manager. Direct supervision and evaluation of Home Health Aides and Licensed Practical Nurses are required.This position involves home visits and periodic office meetings.Reliable transportation, a current driver's license, and agency-required liability insurance are necessary.The average number of patients per day is 5-8.The contractor will be assigned between 5-10 patients per day, which equates to roughly 6-8 points.Each type of visit and activity holds a different point value.If the patient census is low, the contractor may float to a nearby clinic to pick up hours.Types of cases include Start of Care, Wound Care, and Follow-up Visits.No first-timers accepted; recent specialty experience is mandatory.Weekend requirements vary.On-call requirements vary.The contractor must be willing to travel up to 75 miles for patient visits.Mileage will be reimbursed at the IRS standard rate.All candidates must be proficient with HomeCare HomeBase and have utilized it within the last six months.If candidates have used it outside of the last six months, retraining may be required.Documentation must be completed within 24 hours of the appointment with the patient.Delayed documentation impacting billing or follow-up patient care may result in a cancellation.There are no guaranteed hours with this contract. Weekly productivity must meet 30 points, equating to 40 hours.All overtime must be pre-approved. Candidates must accept every patient on their schedule, regardless of distance.
