1 year ago Be among the first 25 applicants Position Summary Primary responsibilities include providing outreach and engagement services with individuals, groups, and organizations in the community, such as police departments, schools, churches, DCFS, Department of Public Aid, Courts, etc. Position Responsibilities Provide mobile crisis response services in pairs, including assessment, engagement, and safety planning with individuals in the community. Conduct outreach/engagement follow-up visits and calls to Call4Calm or 988 inquiries; contact clients from health plans or recently discharged from hospitals or ERs; provide care coordination and referrals to additional programs as needed. Maintain regular communication with referral sources to coordinate treatments and relay case progress, questions, or concerns regarding services or options. Promote HRDI programs to hospital personnel and develop contacts for future referrals. Distribute marketing resources to community partners explaining HRDI programs, treatment options, and requirements to potential clients. Complete required paperwork related to screenings and submit reports as requested. Provide staffing for shifts and on-call coverage as needed. Communicate effectively with the Program Supervisor and team for consultation and information sharing. Complete all documentation and reports timely and thoroughly. Perform other duties or projects as assigned. Participate in collaborative projects and meetings with referral sources to facilitate outreach objectives. Identify and evaluate referral trends and participate in program/process improvements. Perform initial case management to facilitate program admission and retention, encouraging client participation and follow-up with agencies and insurance carriers. Qualifications High School Diploma or GED; at least 1 year of relevant experience. Knowledge of community associations/affiliations. Excellent communication and writing skills. Well organized, detail-oriented, and able to work in a fast-paced environment. Knowledge of computers and applicable software. Valid Driver’s license. Ability to lift 20-30 lbs.
