Case Manager Job Description
The Black Mental Health Corporation strives to provide excellent, quality mental health care to underserved communities. It is the goal of The Black Mental Health Corporation to rebuild these communities one family at a time through excellent mental health and social assistance services. TBMHC was created by Desmond Yanamayu with people of color in mind. After realizing this diverse population experienced challenges and struggles that were unique from many other racial/ethnic groups. Because of this, he wanted to create a space where black people could feel comfortable getting counseling with someone who understood issues specifically related to them. Even still, The Black Mental Health Corporation's services are open to people of all ethnicities and racial backgrounds. A Case Manager is responsible for helping those with mental health issues or those experiencing life-altering circumstances to gain access to rehabilitation programs and other community services. Their duties include meeting with clients and their families, connecting clients with professional services and maintaining consistent communication with clients to assess their wellbeing and make changes to their treatment plan as needed. Essential Duties and Responsibilities: As a Case Manager you will work collaboratively with the clinical team to engage, educate, communicate, and coordinate care with clients, their families, behavioral health, medical and dental providers, community resources and others in ensuring that all services prescribed in the individualized service plan are implemented. You will perform all case management functions associated with caseloads including outreach and follow-up, staffings, client transportation, completing assessment and service planning processes and identifying the client's need for further or specialty evaluations. You will be responsible for maintaining the client's comprehensive clinical record, including clinical documentation (assessments, provision of services, coordination of care, discharge planning, etc). You will provide continuous evaluation of the effectiveness of treatment through the ongoing assessment of the client and input from the client and relevant others resulting in modification to the service plan as necessary. Skills / Requirements: A High School Diploma/GED and three years of relevant experience in a behavioral health setting; An Associate's degree and two years of relevant experience in a behavioral health setting; OR A bachelor's degree in the behavioral health field; OR A non-behavioral health field bachelor's degree and one year of relevant experience in a behavioral health setting. Must be at least 21 years of age. Background check. Minimum typing speed of 25 wpm. Must have your own working vehicle and be willing to transport clients. Full coverage Car Insurance. Current Ohio Driver's License and a current satisfactory Motor Vehicle Record (MVR). Job Type: Part Time/Full-time Location: Cleveland, Columbus, & Toledo Schedule: Flexible. May require weekends.
