The Hospitalist-Nurse Practitioner is responsible for the plan of care for patients in acute care and swing-bed settings. The successful candidate will work closely with the Hospital Medical Director, hospital nurses, the Case Manager and family practice providers to ensure exemplary patient care is always maintained. As a credentialed and privileged provider, the Hospitalist-Nurse Practitioner provides medical care based on the scope of practice and works primarily within the hospital setting. The Hospitalist-Nurse Practitioner will ensure that patients are taken care of by managing patient rounds, along with hospital admissions and discharges.
PHC is looking for a Hospitalist-Nurse Practitioner who is flexible, a good communicator, is able to adapt to dynamic environments and has a patient-focused mindset.
Primary responsibilities may include, but are not limited to the following: Examine patients and review medical history upon admission. Order testing and interprets results to determine plan of care. Provide immediate treatment based on diagnostic testing results utilizing standard of care. Communicates and collaborates with a multidisciplinary clinical team including emergency services staff to ensure exemplary patient care. Works closely with Case Manager and primary care providers to coordinate transitions of care or discharge services. Responds immediately to patient needs or changes in conditions as assessed by nursing staff and is readily available during on-call hours. Ensures continuity of care, efficient throughput, consistent communications, and an exceptional patient experience for every patient. Assists with compliance to safety, environment, infection control, quality improvement, and other regulatory requirements. Responsible for patients admitted to inpatient, outpatient observation, swing bed skilled and swing bed intermediate. Updates patient information and electronic medical records with latest findings and treatments. Provides medical care in accordance with hospital policies and procedures. Participate in the development of patient care protocols. Maintains good standing with licensure and malpractice liability insurance. Participates in periodic chart reviews. Provides support to the Emergency Room in the event of a trauma situation.

Kansas Board of Nursing Licensure as an APRN
Advanced Cardiac Life Support Certification
Advanced Trauma Life Support Certification
Pediatric Advanced Life Support Certification
Drug Enforcement Agency Registration
