Summary
The Associate Director of Continuum of Care Programs (ADCCP) supports the Director of Continuum of Care Programs in the oversight and implementation of CFH’s Rapid Re-Housing (RRH) and Supportive Services Only (SSO) programs. The ADCCP will supervise key program staff, assist with program planning and performance monitoring, and ensure effective service delivery across both programs. They will serve as a primary point of contact for funders and external partners, strengthening collaborations within the NYC Continuum of Care (CoC) network. This role reports directly to the Director of Continuum of Care Programs and plays a critical part in ensuring compliance, quality assurance, and the successful integration of services within CFH and the broader community.
Responsibilities
Support the Director of Continuum of Care Programs in recruiting, orienting, training, and supervising staff within the CoC team. Assist in overseeing quality assurance for CFH’s Rapid Re-Housing (RRH) and Supportive Services Only (SSO) programs; support in tracking, compiling, and reporting program data on a monthly basis. Provide direct supervision and support to assigned staff; assist in conducting employee performance evaluations and implementing performance improvement plans as directed. Collaborate with senior leadership and community providers to support cost-effective operations and promote best practices and standardized processes across programs. Assist in implementing policies and procedures that coordinate CFH’s services with community health facilities and social service agencies. Coordinate with management and CFH Operations to ensure effective integration of RRH and SSO services within health and residential programs. Monitor program expenditures and support the Director in ensuring alignment with approved contract budgets. Serve as a secondary liaison with funders, including HUD and CoC stakeholders, to support contract compliance and program performance. Participate in and help facilitate program supervision, case conferences, staff meetings, and internal trainings. Monitor documentation and data collection requirements to ensure compliance with funder mandates. Assist the Director with reporting to HUD, CoC, and CFH senior management as needed.
EDUCATION:
Bachelor’s Degree Preferred, LCSW preferred
Experience
A minimum of 3 years work experience in a client-facing, nonprofit organization preferred. 1-2 years of experience in HUD programming within a health services agency preferred. 2-3 years of supervisory experience in a social service setting. Familiarity with working in the fields of substance abuse, mental health, and/or homelessness. Previous experience with CARES/AWARDS, SAGE and eClinical works, preferred.
Skill Sets
Excellent organizational and communication skills required. Ability to multi-task and meet assigned deadlines from a variety of management staff. Ability to work in a fast-paced and ever-changing environment. Leadership and/or facilitation experience with a group or team Public presentation and speaking skills Bilingual Spanish is strongly preferred. Extremely proficient in Microsoft Office Suite; the employee is willing and able to: Create, edit, and interpret charts, tables and spreadsheets (via Microsoft Excel). Effectively manage, filter and sort data (via Microsoft Excel). Create well-organized documents and outlines (via Microsoft Word). Manage mailbox and calendar (via Microsoft Outlook).
