Job Description
The Clinical Social Worker MSW is a master's level social work professional who provides interventions to remove psychosocial barriers preventing safe and timely transition through the continuum of care. The MSW collaborates with members of the healthcare team, patients/families, and community liaisons to ensure psychosocial and socioeconomic needs are addressed, enhancing outcomes and patient satisfaction.
Ideal Candidate
Holds a Master's Degree in Social Work. 2 years of recent experience working as a Social Worker in an acute care/hospital setting. 2 years of recent experience working as an MSW in Care Management of a hospital. 1 year of recent experience placing behavioral health patients in an acute care setting. 2 years of recent experience working directly in Hospital Discharge Planning and Care Coordination.
Required Qualifications
Master's Degree in Social Work. 1-3 years of recent experience in an acute care inpatient setting with a patient population in the area of service. 6 months of advanced communication skills to effectively interact with patients, hospital staff, physicians, and the community. Computer experience/literacy as needed for the job.
Preferred Qualifications
6 months of knowledge of community resources with care coordination and discharge planning experience preferred.
