We are seeking a dedicated and compassionate Admission/Discharge Nurse to join our team. This role will begin as an on-site position with a primary focus on coordinating patient admissions and discharges with the opportunity for this role to transition to a virtual on campus position in the coming months.
Evening shift 3pm- 11pm
Job Summary:
Administers care and education to patients and families through a virtual platform. Provides resources, education and leadership to the nursing caregivers and communicates interdepartmentally with the care team. The Connect Care Nurse will be located on the Good Samaritan campus and be an integral part of the patient care team.
Essential Job Duties:
- Assess patient need for self-care instructions related to admission diagnosis or chronic disease process.
- Collaborates with care team to develop the plan of care for patients requiring education and/or intervention.
- Initiates instruction and promotes self-care.
- Implements strategies to provide patient, significant other, and staff participation in health promotion.
- Utilizes age specific criteria to provide education.
- Documents in designated sections of computerized record (i.e. education, discharge).
- Maintains communication both intra and interdepartmentally for continuity of care and patient education.
- Seeks educational resources and creates learning experiences to enhance and maintain current knowledge and skills for his/her competence in nursing practice and individual professional growth.
- Performs other duties as needed or assigned.
- Education: Graduate from an accredited school of nursing. Certification preferred.
- Experience: Graduate of RN Program
Two years of experience in perspective specialty
Computer skills including Microsoft, PowerPoint, Word, Excel
- Licensure: Current Registered Nurse licensure in the state of Indiana
Basic Life Support certification

A. Reports to the Nurse Manager and/or Director of the department.
