Job Summary
As a Home Health Physical Therapist, you will plan, organize, and deliver physical therapy programs that assist clients in recovering mobility, relieving pain, increasing strength, and preventing deformities. Your responsibilities will include:
- Conducting examinations and evaluations for clients affected by injury or disease.
- Utilizing tools and techniques for physical therapy interventions and documenting client information for evaluation.
- Establishing and modifying therapeutic plans as needed.
- Educating clients and their families about treatment plans, schedules, and self-exercises to be continued at home.
- Applying age-related evaluations and treatments catering to diverse client needs.
- Demonstrating flexibility and readiness for variable work schedules and assignments.
- Participating in departmental promotion activities and maintaining communication with other departments and stakeholders.
- Supervising assistants and providing ongoing training and education to peers.
This role may involve working with individuals of all ages receiving intervention in post-acute, long-term care, and home health care environments.
Qualifications
- Must possess appropriate education as per state licensure.
- State licensure as a Physical Therapist is required.
- Valid driver’s license may be necessary.
- Required to obtain and maintain department-specific competencies and certifications, including Basic Life Support (BLS) for healthcare providers within the first 60 days of employment.
