We’re looking for a
Flex/Per Diem/PRN Hospice Nurse Practitioner
to join our team in
Marion, IN . You will report directly to the Executive Director or Administrator and be responsible for performing Face-to-Face (F2F) encounters, gathering clinical information to support hospice diagnosis and prognosis, and communicating findings to the hospice physician for certification or recertification of hospice eligibility.
The scope of practice for the hospice NP is limited to tasks necessary for hospice F2F visits, including:
Performing physical assessments within CMS regulatory time frames.
Reviewing medical treatment plans, diagnostic tests, lab results, and medication lists with the physician.
Making recommendations to the IDG based on F2F assessment findings.
Notifying the hospice agency if unable to complete a F2F visit as scheduled.
About You
Master's or higher degree in Nursing.
Completion of an accredited graduate-level nurse practitioner program.
Current state RN license.
Current state license or certification as an advanced practice nurse.
Current national certification as a Nurse Practitioner (ANCC or AANP).
Must have a collaborative agreement in place.
