Position Summary:
LEAP is seeking an experienced, compassionate, and organized Adult Community Case Manager to join our Case Management team in Farmington, Maine. In this role, you will coordinate essential services and support for adults with intellectual- and developmental disabilities and/or autism. You will serve as a trusted advocate, helping individuals access resources, navigate MaineCare and waiver systems, and develop and implement person-centered plans that reflect their goals and dreams.
This position involves a dynamic mix of community-based work, family engagement, and cross-disciplinary coordination. If you are passionate about person-centered care and making a meaningful difference, we want to hear from you.
Requirements
Key Responsibilities:
•Coordinate, monitor, and document individualized services based on Person-Centered Plans (PCPs).
•Facilitate assessments across medical, housing, educational, vocational, and social domains.
•Ensure regulatory compliance with MaineCare Waivers, Evergreen documentation, and state/federal guidelines.
•Conduct home and program visits, maintain monthly contact, and respond to health and safety issues promptly.
•Advocate for individual rights and access to benefits such as MaineCare, Social Security, and food assistance.
•Participate in planning meetings, quality assurance reviews, and crisis interventions.
•Maintain timely and accurate records using case management software (Evergreen).
•Foster professional, respectful relationships with individuals, families, and service providers.
•Support team collaboration, participate in ongoing training, and contribute to LEAP's mission.
Qualifications:
•Bachelor's degree in a related social services field
•Minimum of one year of full-time adult developmental services case management experience in the last 5 years strongly preferred; one year of full-time recent, related experience will be considered.
•Valid Maine Driver's License with a driving record that meets LEAP's policy
•Familiarity with adult developmental services, Mainecare waivers, and adult protective services
Skills & Competencies:
•Strong knowledge of local resources and service coordination.
•Clear, respectful communication and high-quality documentation & writing skills.
•Effective time management and organizational skills, including the ability to manage multiple priorities with accuracy, professionalism and empathy.
•Proficiency with Microsoft Office and case management software (e.g., Evergreen).
•Commitment to dignity, equity, and meaningful inclusion for people with disabilities.
Physical Requirements:
•Ability to sit, stand, walk, bend, stoop, kneel, and lift up to 25 lbs. •Regular travel using personal vehicle to community and home visits.
Work Environment:
Flexible, hybrid position providing options for both remote work from home & in-office work. LEAP's Central Office is located in Farmington, and the case management team supports individuals across Oxford, Franklin, Kennebec, Androscoggin and Somerset counties. Frequent travel required, including visits to homes and community settings.

Salary Description
$50,960 to $65,520
