Substance Use Outpatient Counselor Case Manager.
Provide individual, group, and family counseling services for clients experiencing SUDs and substance- related issues in a clinical, culturally sensitive, and trauma-informed manner for our Elgin Outpatient substance use treatment program.
Schedule
: Tuesday through Friday from 10:00am to 6:30pm and Sunday from 9:00am to 5:30pm

Essential Functions
Provide a continuum of counseling services to individuals with alcohol/drug, co-occurring, or gamblingrelatedconditionsand/orproblemsusingavarietyoftherapeuticmodalitiesandevidence- based practices. Coordinate and facilitate group and individual counseling sessions (ASAM Levels 0.5, 1, 2.1, continuing care), DUI, and psychoeducation. Responsible for an assigned caseload and the associated documentation including treatment planning,insuranceauthorization,continuingcare,casemanagementanddischargeplans.Monitor progress and participants’ response to treatment. Communicateprofessionallyandconsistentlywithreferralsourcestoprovidestatusreportsand completion/discharge documentation for mutual clients. Completebiopsychosocialassessment,alcohol/drugevaluations,andDUIevaluationsusingDSM- and ASAM criteria. Maintainweeklyperformance productivity. Ensureadherencetoethicalstandardsoftreatment,includingNASWCodeof Ethics. Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients,families,otherLSSIworkforcemembers,andcollaborativeorganizationsand/orindividualI adignified,respectful,honestandfair manner.
Position Qualifications
Highschooldiplomaorequivalent required. Bachelor’sormaster’sdegreeinsocialwork,psychology,counselingorrelatedfieldstrongly preferred. One(1)tothree(3)yearsofexperienceinsubstanceusefield preferred. Backgroundcheckclearance required. Professionalcertificationorlicensure(LSW,LPC,LCSW,LCPC,orCADCrequired,ormustobtain within two (2) years of hire date). AnnualTBtest required. KnowledgeandexperienceusingDSM-5andASAMcriteriatomakeaccuratediagnosisof substance use disorders and treatment/intervention recommendations required. Demonstratedabilitytorecordandproofreadinformationonformsandcharts required. Experienceprovidingtelehealthservices preferred. Demonstratedabilitytoadheretoprofessionalstandardsofethicsandmaintainprofessionalismat all times. Familiaritywithaccreditationandlicensurestandards preferred. Demonstratedabilitytocommunicateinaclear,comprehensiblemanner,bothverballyandin writing. Excellentorganization,presentationandpc/computerskills,includingexperienceusingMicrosoft Office (Outlook, Teams, Word, Excel, PowerPoint) along with other related software. Validdriver’slicense,ingoodstandingforthestateofresidency preferred. Accesstoreliabletransportation preferred. ValidILstatutoryminimumliabilityinsurancecoverage,bodilyinjuryandpropertydamage preferred.
