Key Responsibilities : Conduct comprehensive assessments and develop individualized care plans for geriatric patients. Provide ongoing medical care, including diagnosis, treatment, and management of chronic and acute conditions. Collaborate with interdisciplinary teams to coordinate care and support services for patients and their families. Educate patients and caregivers on health management, medication adherence, and preventive care. Maintain accurate and up-to-date medical records in compliance with healthcare regulations.
