Job Summary
The Care Coordinator/Clinical Case Manager (CC) role is a core member of the Care Team and focuses on coordinating all aspects of health and human services related to patient care. This position will serve as the point of contact for their assigned care team and for the panel of relatives served by the team. The CC will assist with our patient's needs including, but not limited to, internal/external linkages to care, scheduling, document support, system navigation, referral tracking/follow-up and communications.
Key Responsibilities
- Coordinate all aspects of health and human services related to patient care.
- Serve as the point of contact for the assigned care team and their panel of relatives.
- Assist with patient needs including linkages to care, scheduling, and document support.
- Facilitate system navigation and referral tracking/follow-up.
- Maintain effective communication within the care team.
- Background check, health screen, and drug screen clearance are mandatory.
- 1-2 weeks of training depending on experience.

Eligibility for health benefits is based on verifying that an average of 30 hours per week during the first 4 weeks of the work assignment has been met. If you meet eligibility requirements and take action to enroll, you will be covered no earlier than 60 days into your assignment, depending on plan selection(s)).
- 401k (Eligible on the first 401k open enrollment date following 6 consecutive months on assignment. 401k Open Enrollment dates are 1/1, 4/1, 7/1, and 10/1).

This position is subject to a background check based on its job duties, which may include patient care, working with vulnerable populations, access to financial and confidential information, driving, working with heavy machinery, or working in a warehouse or laboratory environment.
