Job Summary
The Care Coordinator/Clinical Case Manager (CC) role is a core member of the Care Team and focuses on coordinating all aspects of health and human services related to patient care. This position will serve as the point of contact for their assigned care team and for the panel of relatives served by the team. The CC will assist with our patient's needs including, but not limited to, internal/external linkages to care, scheduling, document support, system navigation, referral tracking/follow-up and communications.
Key Responsibilities

• Coordinate all aspects of health and human services related to patient care.
• Serve as the point of contact for the assigned care team and their panel of relatives.
• Assist with patient needs including linkages to care, scheduling, and document support.
• Facilitate system navigation and referral tracking/follow-up.
• Maintain effective communication within the care team.

Qualifications

• Background check, health screen, and drug screen clearance are mandatory.

Additional Requirements

• 1-2 weeks of training depending on experience.
