Schedule:
Part-time (24 hours/week, Monday-Friday, daytime hours)
Are you a passionate and skilled LICSW or LMHC looking to be at the forefront of innovative patient care? Primary care settings are increasingly the first point of contact for individuals facing behavioral and mental health challenges. Our organization champions the Collaborative Care Model, embedding dedicated Behavioral Health Clinicians like you directly within these essential primary care offices. You'll become an integral part of a dynamic, multidisciplinary team, making a tangible impact by bridging the gap between physical and mental wellness.
As an Embedded Behavioral Health Clinician, you will partner closely with primary care providers to recognize, assess, and manage patients experiencing mental health disorders and substance use concerns. Your expertise in delivering short-term, evidence-based therapy will empower patients, helping them navigate their challenges and improve their quality of life. What You'll Do (Key Responsibilities):
Comprehensive Patient Assessment: Proactively review medical records and conduct thorough behavioral health evaluations to identify patient needs, assess for mental health disorders, and evaluate the continuum of substance use (from use to dependence). Evidence-Based Therapeutic Interventions: Utilize your clinical skills to provide brief treatment approaches, including Cognitive Behavioral Therapy (CBT), Motivational Interviewing, Problem-Solving Therapy, Supportive Therapy, Behavioral Activation, and Psycho-Education. Collaborative Treatment Planning: Partner with the primary care team to design and implement person-centered behavioral health treatment plans based on comprehensive assessments and patient goals. Care Coordination & Resource Navigation: Conduct behavioral health screenings to determine appropriate levels of care, identify necessary community resources, and facilitate referrals. Staff & Provider Education: Develop and deliver effective educational sessions and curricula on behavioral health and substance use topics for primary care staff and fellow providers. Timely & Accurate Documentation: Complete all clinical documentation meticulously and within expected timeframes, adhering to all organizational policies and compliance standards. Insurance Navigation: Maintain knowledge of, or demonstrate the ability to quickly learn, insurance standards for commercial plans and MassHealth products. Professional Growth & Supervision: Actively participate in regular supervision sessions to discuss clinical cases, organizational developments, agency updates, and receive feedback for performance enhancement. Building Key Relationships: Cultivate and maintain effective working relationships with physicians, patients, visitors, external providers, regulatory bodies, and the broader healthcare community. Upholding Confidentiality: Strictly maintain patient confidentiality and protect all sensitive data in accordance with privacy regulations and organizational policies.
What You'll Bring (Qualifications & Skills):
Education: A Master's degree in Social Work or another directly-related Human Service discipline. Licensure: Current, unrestricted Massachusetts license as an Independent Clinical Social Worker (LICSW) or a Licensed Mental Health Counselor (LMHC) is required. Language: Spanish speaking highly preferred Collaborative Excellence: A proven ability to work effectively and harmoniously within a multidisciplinary team setting. Communication Prowess: Strong, proactive verbal and written communication skills, with the ability to engage effectively with diverse individuals. Independent & Flexible: Demonstrated ability to work autonomously, manage time effectively, and adapt to the evolving needs of a primary care environment. Patient-Centered Approach: A genuine passion for making a positive difference in the lives of the clients we serve. Clinical Acumen: Solid understanding of evidence-based treatment modalities and their application in brief therapy models.
