We are currently seeking

Fee-for-Service (FFS) Field Registered Nurses (RNs)

to perform in-home visits across

all five boroughs of New York City .

Position Overview:

The Field RN plays a key role in delivering

skilled nursing care , conducting

initial and follow-up assessments , and coordinating with multidisciplinary teams to ensure patients receive comprehensive, quality care.

Responsibilities:

Conduct nursing assessments and evaluate patients’ physical and psychosocial needs Educate patients and families on disease management, medication adherence, and prevention Develop and implement individualized care plans in collaboration with healthcare teams Document all visits and services in a timely and accurate manner to maintain compliance Provide oversight and instruction to caregivers and home health aides as needed Perform other related duties as assigned by clinical leadership

Qualifications:

Active New York State Registered Nurse (RN) license 1–2 years of experience in home care (CHHA) or long-term care preferred Strong clinical and assessment skills Must carry professional liability insurance Must have access to a reliable vehicle for travel throughout NYC

Compensation (Fee-for-Service Rates):

Initial Assessment Visit: $130 Reassessment Visit: $100 Resumption of Care Visit: $100 Supervisory Visit: $80

Flexible Per Diem Schedule
