Hoag Health System Primary Duties and Responsibilities
The Physical Therapist I assesses individual patients Physical Therapy needs, and then establishes and implements a treatment program. This role reviews the medical history, applies screening tests, and meets with patient and family to determine clinical needs, impairments, functional limitations, and disabilities. Determines a plan of care to include appropriate interventions, expected intensity, duration of therapy, and recommendations for discharge. Job Description
Primary Duties and Responsibilities
The Physical Therapist I assesses individual patients Physical Therapy needs, and then establishes and implements a treatment program. This role reviews the medical history, applies screening tests, and meets with patient and family to determine clinical needs, impairments, functional limitations, and disabilities. Determines a plan of care to include appropriate interventions, expected intensity, duration of therapy, and recommendations for discharge.
Provides education to patient, family, and/or caregiver regarding purpose of activity, use of equipment, home exercise and activity. Performs regular re-examinations, measures outcomes, and seeks feedback from patient, family, and patients’ interdisciplinary medical team. Oversees the clinical work of the Physical Therapist Assistant and Rehabilitation Aide. Manages multiple patients at the same time and provides care in multiple clinical settings and locations.
Provides services and required documentation timely, accurately, and in accordance with professional and regulatory standards. Participates in outside continuing education and incorporates information into practice. Performs other duties as assigned.
Qualifications
Education and Experience
Previous experience with Microsoft Office and healthcare database systems preferred. Bilingual or multilingual preferred.
Additionally, Specific Department Qualifications Are As Follows
Acute Rehabilitation Unit: Demonstrates competency in Inpatient Rehabilitation Facility Quality Reporting Program (IRF QRP). May require travel to patient’s home for home evaluation. Pelvic Health Program: Training/experience in completing internal pelvic floor exams and treatment. Required to spend monthly clinical time with a PT IV and required to present an annual in-service case presentation, or literature review to the department to enhance department collective knowledge and collaboration. Rehabilitation Services: Fulfills mandatory stroke education requirements per certification agency.
License Required
Current licensure as a Physical Therapist in the State of California, or eligible and applied for licensure as a Physical Therapist in the State of California. If the candidate has applied for licensure in the State of California a 6-month grace period is allowed to receive the licensure.
