The Hebrew Home at Riverdale serves over 18,000 frail and disabled individuals, empowering them to live independently in their homes for as long as possible.
This is a dual position that is responsible for the day-to-day program coordination for the SNALR model and senior case management functions. As a program coordinator, he/she is responsible for all planning, development, implementation, evaluation and improvement of SNALR program services and the delivery of high quality services. During the absence of the administrator, is empowered to act on behalf of the administrator concerning the operation of the facility and the health, safety, comfort and well-being of the residents. With the mission to promote "aging in place," the Senior Case Manager position will holistically address and support the resident's medical, social and community needs. This role implements the facility's policies and procedures, which create a case management system that provides for initial and ongoing assessment of a resident's strengths and needs. As well as oversees and coordinates a Individualized Service Plan (ISP), provides or arranges for the provision of identified services, monitors the receipts of such services, and evaluates their effectiveness in improving resident functioning, quality of life and documents the activities and outcomes
Qualifications:
- Bachelor's Degree required in a human services related field or its equivalent from a four-year college (BA/BS/BSW) with a minimum of two years' experience and /or training in providing services to older adults and/or residents who have dementia; or four to ten years related experience; and/or training or equivalent combination of education and experience.
- Formal education must have included completion of a course of study in one of the following areas but not limited to: social work programs, gerontology studies, human services for dependent adults, human resources or home economics. Related work experience may include but need not be limited to residential care, adult care facilities, rehabilitation, health care management and social work.
- At least 2 years of supervisory experience
- Annual salary: $70,000 - $80,000
