Your Role

Emotional Support & Counseling: Assess psychosocial needs and provide compassionate counseling and crisis intervention. Care Planning: Collaborate with nurses, physicians, and other team members to create individualized care plans. Family Education & Advocacy: Help patients and families understand resources and navigate complex systems.

What We’re Looking For

Education: Master’s Degree in Social Work (MSW) required. Experience: Prior experience in healthcare, hospice, or palliative care settings preferred. Skills: Strong interpersonal, organizational, and communication skills. Able to work independently. Requirements: Valid LCSW license in California NPI number Reliable transportation, valid driver’s license, and auto insurance Ready to Join a Mission-Driven Team?
