to practice team-based care in our progressive, southside family medicine clinic in Lafayette, IN (Home of Purdue University).

Employment Opportunity Details:

Full-time (4 days per week schedule) Collegial team environment with participation in process improvement 100% outpatient Onsite procedure suites and lab Traditional family medicine practice: chronic disease management, wellness, for all ages of patients Shared phone call only: approximately 1:25 for FM 1:12 IM Excellent base salary plus incentive opportunities and sign on bonus Comprehensive benefit profile including relocation bonus

Qualifications: US BE/BC in Family Medicine, Internal Medicine or Internal Medicine/Pediatrics MD/DO Indiana license or eligible Consistent and consecutive employment/training history Excellent communication skills Ability to provide high quality patient care Team oriented, positive and patient centered Commitment to lifelong learning and open to change/improvement Demonstrated connection to our Values - Compassion, Excellence, Team and Purpose
