General Clinical
Obtain health history and perform physical exams Diagnose, treat, counsel, and coordinate patient care Update problem list and care plan at each visit Complete all progress notes within 24 hours of patient visit Respond to on-call calls within 20 minutes Prescribe medication per Illinois law and guidelines Follow established quality and clinical standards Meet Meaningful Use standards using certified EHR technology Manage EHR In-Basket, respond to messages within 72 hours Ensure activities meet reimbursement guidelines Provide culturally competent care Involve families in care as appropriate Encourage patient self-management and education Coordinate with care teams and case management Transfer patients appropriately for higher acuity cases PCMH Requirements
Coordinate with health center staff to meet patient needs Participate in huddles and meet productivity standards Ensure timely and appropriate patient care involving staff Provider Development
Participate in staff orientation and training Ensure care team meets internal certification standards Participate in proficiency testing May serve as a preceptor for training programs Adhere to HRSA and FTCA regulations Perform other duties as assigned Requirements/Preferences
Current Illinois PA license and CME requirements Board certification Obtain and maintain collaborative agreement and DEA license Preferred: 2+ years outpatient primary care experience Knowledge of quality assurance, safety, infection control Electronic Health Records proficiency; EPIC preferred Cultural competency and interpersonal skills Organizational skills Working Conditions/Equipment
Ability to concentrate amid interruptions Stand, sit, walk up to 8 hours Ability to lean and stretch during exams Computer/laptop use Note: Candidates may need to be vaccinated against communicable diseases or request exemptions.
