OPEN DATES: Sept 1 – 7 September 15-21 September 22 – 28 October 6 – 12 October 13 – 19 October 20 -26 October 27 – November 2 November 3 – 9 December 1 – 7 December 15 – 21 December 29 – January 4 JOB for Coverage: no available Practice Setting: Hospital based.Average Number of Cases Per Day: 6 Trauma Level/Designation: none Annual ER Volume: 2Documentation System/EMR: EPICPatient Age Demographic: allWhat Percentage of Practice is Trauma Hospital Based Clinic Based Surgical Non-Surgical: 10 Surgical Procedures Required: Thyroidectomy Extended Parotidectomy General Surgical Otolaryngology Requirements: Tonsillectomy Adenoidectomy PE Tube General Non-Surgical Otolaryngology Requirements: Hearing Evaluation Eval and Manage Disorders Involving Ears Nose Throat and Skeletal Facial Structures Endoscopic Procedures Required: Septal Sinus Bronchoscopy - Flexible Bronchoscopy - Rigid Laryngoscopy Facial Surgery Procedures Required: Rhinoplasty Facial Fractures Skin Grafts/Flaps Facility Reimburse for the Following Costs: Flight Hotel Rental Car Mileage Billed at the IRS Standard Rate ~Estimated Credentialing Timeframe: 120 Schedule 24Hour, 24 hour call. 7am to 7am.
