Iredell County Government $52,997.53 - $82,676.15 Annually Statesville, NC Full-Time Continuous Responsibilities
This position is responsible for case management of two programs within Adult Services. The programs provide services to disabled adults age 18 and older, and are for Medicaid eligible recipients. The first program is the Community Alternatives Program (CAP) which provides home and community-based services to adults who are at risk of institutionalization in a nursing home. The second program is the Special Assistance In-Home program which provides essential support to low-income individuals who are at risk of entering residential facilities, and a cash supplement and additional support services enable them to remain in the home. Case management for both programs require comprehensive assessments which evaluates psycho-social, economic, environmental, physical and mental health factors. The social worker develops a plan of care, mobilizes resources and monitors the plan of care for the patient in their own home. There is collaboration with multiple community professionals including medical personnel and community-based providers. The social worker advocates for the client and also serves as the liaison with Adult Medicaid staff. The social worker provides service intake several times monthly which includes submitting CAP referrals and the ability to educate the community on adult service programs and community resources. This position also requires participation in the energy programs with processing applications to determine eligibility. This position requires in home visits as well as seeing clients in the office. Other duties may be assigned as agency needs require. Special Attributes & Skills
Organizational skills, ability to prioritize, multi-task and meet deadlines. Effective communication skills both oral and written. Critical thinking and problem solving for assessment and crisis intervention. Computer literacy to maintain and manage case records. Medical terminology and knowledge of community resources is a plus. Ability to work effectively with physically and mentally disabled adults, elderly adults and their families. Ability to work as member of team. Must have valid driver's license and automobile for in and out of county travel. Minimum Education and Experience
