nRESPONSIBILITIES:
n· Patient Care & Service - Promote and restore patients' health by completing all activities listed in the care plan; collaborate with Community Health Nurse/Therapist; perform various activities such as personal care, light housekeeping, meal preparation and errands.
n· Communication - Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularly.
n· Documentation - Accurately and completely document client care on activity sheet.
n· Compliance - Adhere to required department and system protocols, HIPPA regulations, local, state, federal requirements.
nREQUIRED QUALIFICATIONS:
n· NYS DL - valid New York State Driver's License - New York State Department of Motor Vehicles (NYSDMV)
n· HHA - Home Health Aide certification - New York State Education Department (NYSED)
nPREFERRED QUALIFICATIONS:
n· Compassionate, patient focused and dependable professional.
n· Critical thinking skills, decisive judgment and the ability to work with minimal supervision.
n· Exceptional documentation and communication skills..

nPHYSICAL REQUIREMENTS:
nM - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
