Job Title: Home Health Aide I (HHA)

Hours Per Week: Full-Time & Part-Time available
Schedule: Monday - Friday; Days, 8am-5pm

RESPONSIBILITIES:
- Patient Care & Service: Enhance patients' health by following the care plan; collaborate with the Community Health Nurse/Therapist; perform essential activities including personal care, light housekeeping, meal preparation, and errands.
- Communication: Maintain clear communication regarding patient health status, treatment plans, and progress; engage regularly with office and supervisory staff.
- Documentation: Ensure accurate documentation of client care on activity sheets.
- Compliance: Follow all department protocols, HIPAA regulations, and local, state, and federal requirements.
REQUIRED QUALIFICATIONS:
- NYS DL - valid New York State Driver's License from NYSDMV.
- HHA - Home Health Aide certification from New York State Education Department (NYSED).
PREFERRED QUALIFICATIONS:
- Compassionate, patient-focused, and dependable.
- Strong critical thinking, decisiveness, and ability to work independently.
- Exceptional communication and documentation skills.
- Ability to read, write, and speak English, and follow verbal and written instructions.
EDUCATION:
Relevant educational background in healthcare is a plus.
LICENSES / CERTIFICATIONS:
HHA - Home Health Aide certification from NYSED, NYS DL - valid New York State Driver's License from NYSDMV.
PHYSICAL REQUIREMENTS:
Medium Work - Requires exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or up to 10 pounds constantly. Involves walking, standing, or squatting regularly.
