Job Title: Certified Home Health Aide (HHA)

Department: Home Care
Location: Monroe County, In the community
Hours Per Week: Full-Time & Part-Time available
Schedule: Monday - Friday; Days, 8am-5pm

Join Our Team! As a Certified Home Health Aide, you will play a critical role in supporting the health and well-being of our patients in the community. Your compassionate care helps restore and promote patients' health through various activities and services.

Key Responsibilities:

  • Patient Care & Service: Implement and follow personalized care plans; assist with personal care, light housekeeping, meal preparation, and errands.
  • Communication: Provide regular updates on patient health, treatment plans, and progress to the healthcare team and maintain open lines of communication.
  • Documentation: Accurately document all client care activities on the activity sheet.
  • Compliance: Follow all department protocols, HIPAA regulations, and relevant local, state, and federal requirements.

Qualifications Needed:

  • Valid New York State Driver's License (NYSDMV)
  • Home Health Aide Certification (NYSED)

  • Good documentation and communication skills.
  • Proficient in reading, writing, and speaking English; able to follow both verbal and written instructions.

Education Requirements:

  • HHA Certification from the New York State Education Department.
  • Valid New York State Driver's License.

Physical Requirements: Medium Work - ability to exert 20 to 50 pounds of force occasionally. Frequent walking, standing, and squatting required.

Compensation: Pay range of $18.65 - $19.75, based on factors including experience, qualifications, and location.
