This position will assess eligibility for long-term support through IDD and provide initial education and community resources to families and individuals who are new to services. This position will develop state plans and will transfer new cases to the Case Manager 2 for the development of a Medicaid state plan, further service coordination, and case management activities. This recruitment may be used to fill future full-time, temporary, and limited-duration positions. This position is a limited-duration position. Limited duration position can last up to two (2) years, but can end at any time. As the Case Manager 1, other duties will include: Review eligibility for long-term support through IDD for individuals who are new to services Distribute resource packet to families and individuals based on eligibility Schedule and complete initial choice advising meeting over phone or at family's home based on Medicaid plan eligibility. Complete state plan and conduct site visit at individuals home to ensure home is safe and individual free from abuse. Review eligibility for OHP and provide information for accessing presumptive Medicaid disability determination Refer individual for Oregon needs assessment once Medicaid determination is complete Maintain a caseload of unfunded plans Refer funding requests to lead Case manager 2 Document visits, meetings, and discussions with eligibility individuals File all initial paperwork, including state plans, return calls, and emails Provide consultation to co-workers of any classification and to staff of partner programs/agencies.
