Family Medicine Physician & Preceptor/Teaching position. Position Summary Provide the full scope of services which fall under the provider’s field of training, including diagnosis, treatment, care coordination, preventive care, and electronic health record (EHR) maintenance, following guidelines related to quality management, risk management, infection control, customer service, and safety programs for Access Community Health Network. Loan repayment potential through the National Health Service Corps (NHSC)
NHSC LRP: Up to $75,000 Full Time for 2 years. NHSC LRP or SUD-W LRP: $5,000 award enhancement for clinicians proficient in Spanish. NHSC SUD-W LRP: Up to $75,000 for 3 years. HPSA Primary Care score 19 for ACCESS Health Centers.
232 Hours of Paid Time Off (PTO) per year & Paid Holidays 40 Hours of CME & $2,000 Reimbursement Malpractice liability provided by the Federal Torts Claims Act (FTCA) Supports H-1B visa and J1 Waivers Flexible schedules and more…! Clinical and Operational Responsibilities General Clinical Obtain health history and perform physical examinations. Diagnose, treat, counsel, and coordinate care for each patient. Review and update problem list and longitudinal care plan at each visit. Meet documentation standards and complete all progress notes within 24 hours of the patient visit. Fulfills on-call responsibilities as scheduled, responding to calls within 20 minutes. Prescribe medications according to Illinois law and professional guidelines. Adhere to quality standards and clinical guidelines established by ACCESS. Meet Meaningful Use standards by utilizing certified EHR technology to improve care quality, safety, efficiency, and health disparities; engage patients; and maintain privacy and security. Manage EHR In-Basket, respond to messages within 72 hours, and transfer responsibilities when appropriate. Patient Support and Education Provide culturally competent care and involve families in care plans. Address patient and family questions and concerns, and promote self-management. Conduct patient education, using appropriate materials and adjusting communication to patient understanding. Coordinate with care team and case management for comprehensive services. Transfer patients with greater acuity appropriately and meet severity of illness admission criteria. PCMH Requirements Coordinate with health center staff to meet patient needs, participate in huddles, and ensure timely, appropriate care. Ensure care team compliance with policies and procedures. Provider Development Participate in staff orientation and training, ensure team meets certification requirements, and engage in proficiency testing. May serve as a preceptor for medical residents, students, and other health professionals. Follow HRSA and FTCA regulations. Other duties as assigned. Requirements/Preferences MD or DO in Family Medicine, with residency completed and board certification. Minimum two years outpatient primary care experience preferred. Current Illinois licensure, DEA, and CME compliance. Knowledge of quality assurance, safety, infection control, and EHR systems; EPIC preferred. Vaccination against communicable diseases may be required unless exempted for medical or religious reasons. Compensation varies based on experience, education, market, and funding.
