Job Responsibilities: Performs assessments and reassessments according to policy and procedure or as warranted by the patient's condition and demonstrates the ability to make sound independent decisions. Initiates, prioritizes, and implements the plan of care and reviews and revises the plan as indicated. Collaborates with the patient, family, and interdisciplinary services and utilizes resources appropriately to support the needs of the organization, patient, and family. Monitors and documents responses to clinical interventions. Facilitates, coordinates, provides, and evaluates education specific to patient and family needs and as appropriate to the care, treatment, and services provided during their stay and at discharge. Delegates tasks to licensed and non-licensed personnel and functions as a mentor/preceptor.
