Development & Implementation: Initiates, prioritizes, and implements the plan of care. Reviews and revises the plan as indicated. Collaborates with patients/families and interdisciplinary services. Utilizes resources appropriately to support the needs of the organization and patients/families. Evaluation: Monitors and documents response to clinical interventions. Education: Facilitates, coordinates, provides, and evaluates education specific to patient and family needs. This includes care, treatment, and services provided during their stay and at discharge. Leadership: Delegates tasks to licensed and non-licensed personnel. Functions as a mentor/preceptor. Demonstrates, encourages, and facilitates teamwork.
