This position requires the ability to demonstrate knowledge and skills necessary to deliver care that is age-appropriate, particularly focusing on adult and geriatric populations. The LPN will work collaboratively with other nursing staff, physicians, and ancillary staff to ensure comprehensive and effective patient care. Key Responsibilities: - Provide both direct and indirect nursing care to assigned residents, following established protocols under the supervision of a Registered Nurse (RN).- Collect and assess patient data related to physical and psychosocial conditions, communicating findings to the RN and interdisciplinary team.- Develop, implement, and revise care plans that are culturally relevant and developmentally appropriate, involving both residents and their families.- Participate in the evaluation of resident outcomes and recommend adjustments to care plans as necessary, ensuring satisfaction and well-being.- Maintain ongoing professional knowledge through continuing education and in-service training.- Mentor and orient new staff as needed, while demonstrating leadership within the nursing practice.- Address safety, cost, and overall impact in the planning and delivery of care.- Adhere to safety practices regarding potential exposure to blood borne pathogens. Qualifications: - Current NY LPN Licensure and BLS certification (AHA or RQI only).- Minimum of 1 year experience as an LPN, preferably in a long-term care (LTC) setting.- Strong communication and collaboration skills.- Ability to provide quality nursing care to a diverse patient population.
