P4 Physical Therapy is looking for a dedicated and skilled Physical Therapist to provide full-time coverage in our outpatient clinic in Birmingham, AL. If you are passionate about providing quality care and looking for a supportive work environment, we encourage you to apply!
Responsibilities include:
- Assessing patients to develop and initiate treatment plans based on the evaluation results
- Reviewing the physician's referral and the patient's condition and medical records to determine the physical therapy treatment required
- Providing direct patient care by established protocols and patient care guidelines
- Documenting findings, progress, and instructions to patients and caregivers
- Being responsible for patient care performed by assistants and rehabilitation technicians
- Completing a written plan of care, developing goals based on evaluation findings, and developing a comprehensive program to attain goals
- Implementing physical therapy treatment program and providing essential physical therapy treatments
- Communicating effectively with patients/caregivers and professional colleagues, including physicians and insurance companies
- Ensuring timely documentation, including daily notes, weekly workers' compensation reports, progress reports, initial evaluations, discharge summaries, and incident reports
Qualifications:
- Graduate from an accredited college with an APTA curriculum
- Current state licensure
- New or recent graduates are welcomed and encouraged to apply
