$2,100 - $2,250 Guaranteed Hours:
36 hours per week Shift:
Monday-Friday, with occasional weekends & holidays; 12-hour shifts Registered Nurse Qualifications
Active Massachusetts RN license (required for all Massachusetts nursing jobs) Minimum 1 year of clinical nursing experience (or 6 months with a BSN) Current CPR certification Successful completion of pre-hire medication test Home health care and IV therapy experience preferred Strong computer skills; BSN preferred Homecare RN Responsibilities
Conduct comprehensive nursing assessments for home health patients upon admission, considering physiological, psychological, and environmental factors Develop, implement, and revise individualized care plans based on assessment findings and case management objectives Utilize HHRG values to determine visit frequency and ensure appropriate interdisciplinary care Refer patients to community healthcare resources as needed Participate in interdisciplinary patient case conferences and evaluate care plan effectiveness Supervise and document Home Health Aide care plans every 14 days Communicate significant changes in patient condition to physicians and care providers Complete all required admission, discharge, and ongoing clinical documentation promptly and accurately Obtain and clarify valid physician orders for all nursing care delivered Plan and coordinate patient discharges, ensuring all paperwork and disposition records are complete Perform additional nursing duties as assigned
