This position is open to current Colorado residents only. This recruitment may be used to fill multiple vacancies, various shifts available. Office of Adult, Aging and Disability Services Colorado Veterans Community Living Center at Fitzsimons Colorado Veterans Community Living Center (VCLC) at Fitzsimons is a beautiful 180-bed, skilled nursing facility serving Colorado's veterans. At Fitzsimons, we provide long-term care, short-term rehab and have a secured memory care neighborhood, and are conveniently located on the medical campus with UC Hospital and the Aurora VA Medical Center. Description of Job The MDS Coordinator position exists to ensure consistent and accurate documentation of 24-hour nursing care provided for residents who have acute/chronic health problems, physical and/or mental limitations, and are unable to live independently or at a more independent level of care. The complete and accurate reporting of this data is of vital importance due to its effect on financial reimbursement as well as quality measures, and both Veterans Affairs and Long Term Care Surveys. This position is also responsible for collaborating with the Nursing Home Administrator to identify, monitor, and improve Clinical Quality Measures throughout the facility. Additionally, as an MDS Coordinator, you can expect to: Make process-level care decisions guided by professional and regulatory standards, tailored to each resident's current condition Complete timely, accurate MDS assessments in compliance with federal and state regulations, with no margin for error Gather, enter, and transmit resident data to the state database; maintain care plans and documentation in the medical record Analyze clinical data for quality improvement using systems such as Abaqis, tracking outcomes in areas like falls, skin integrity, weight loss, pain, mobility, and psycho-social health Document and support Medicare A/B requirements and track resident length of stay Collaborate with the interdisciplinary team to develop individualized care plans and therapy/nursing goals Lead care conferences, coordinate with residents and families, and maintain compliance with scheduling requirements Provide in-service training, policy/procedure updates, and software/regulatory changes related to the MDS process Orient new team members to MDS procedures and facility expectations Coordinate subordinate staff in their delivery and provision of patient care documentation for MDS assessments Other duties as assigned Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights Special Qualifications: RN License and be a Certified Resident Assessment Coordinator Education/Licensure/Certification/Experience: Current, valid licensure as an RN from the Colorado Board of Nursing or any state participating in the Enhanced Nurse Licensure Compact (eNLC) AND Three (3) years of professional experience in an occupational field related to the work assigned to the position Substitutions: Appropriate education will substitute for up to two (2) years of the required experience on a year-for-year basis Conditions of Employment: CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference Checks, Professional License verification (licensure requirements), Drug Screen, PREA (Division of Youth Services), Trails check (direct contact with children), CAPS (direct contact with adults - Mental Health Institutes, Regional Centers, Veterans Community Living Centers) Current, Valid US Driver's License May have to transport residents in state vehicles Vaccinations as required by state and facility Day, evening, and night shifts to include holidays & weekends as needed Travel: May have to transport residents in state vehicles Required to obtain a CPR card Possess and maintain a Colorado RN license in good standing Former State employees who were disciplinarily terminated or resigned in lieu of termination must (1) disclose that information on the application (2) provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position, (3) provide the employee number from the applicant's prior State employment. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination, and failure to provide this information will disqualify the applicant from future State employment with CDHS.
