Description Conduct patient evaluations in inpatient and outpatient settings and craft personalized treatment plans Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function Collaborate closely with a multidisciplinary team, including nurses, physicians, case managers, and other rehabilitation professionals to achieve the best patient outcomes Guide patients and their families through therapeutic objectives, discharge processes, and at-home exercise routines Maintain accurate documentation of evaluations, treatments, and patient progress in accordance with facility standards and regulations Engage actively in care team rounds and interdisciplinary meetings to ensure seamless care coordination Education

Doctorate in Physical Therapy (DPT) or a comparable qualification from a recognized institution Certifications

State licensure as a

Physical Therapist Preferred CPR certification Additional specialized certifications in fields like geriatrics or neurology are advantageous but not mandatory Skills

Previous clinical experience is highly preferred Ability to treat patients with intricate medical and mobility challenges Excellent communication and collaborative spirit Proficiency in digital documentation and clinical charting systems Benefits
