Description Assess patients in both inpatient and outpatient scenarios to tailor personalized treatment plans Implement physical therapy interventions with an emphasis on mobility, strength, balance, and overall function Engage in teamwork with nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes Educate patients and their families about recovery goals, discharge plans, and exercises for at-home practice Document assessments, treatments, and patient progress per institutional and regulatory standards Contribute to interdisciplinary team meetings and collaborate on patient care coordination Education
Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution Certifications
Active state license as a
Physical Therapist CPR certification preferred Additional specialized certifications in areas like geriatrics or neurology are advantageous but not mandatory Skills
Prior experience in therapy practices is highly encouraged A capability to assist patients with complex medical and mobility conditions Excellent communication and collaborative skills Proficient in using electronic health records and clinical documentation systems Benefits
