Description Conduct thorough evaluations of patients in both inpatient and outpatient contexts, tailoring treatment plans to meet their unique needs Implement effective physical therapy interventions, enhancing mobility, balance, strength, and overall functionality Work collaboratively with a team of nurses, physicians, case managers, and rehab professionals to achieve optimal patient outcomes Educate patients and their families on therapeutic goals and provide guidance on discharge planning and home exercises Maintain detailed documentation of evaluations, treatments, and patient progress in alignment with regulatory and facility policies Engage actively in care team discussions and interdisciplinary meetings to ensure seamless patient care Education
Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution Certifications
State licensure as a
Physical Therapist CPR certification is preferred Specialized certifications such as neurological or geriatric are valued but not mandatory Skills
Prior experience is highly favored Ability to manage patients with intricate medical and mobility challenges Exceptional communication and teamwork abilities Familiarity with digital charting and clinical documentation software Benefits
