Responsibilities Conduct assessments of patients in both inpatient and outpatient environments to create tailored treatment plans Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function Coordinate care with nurses, physicians, case managers, and rehab specialists to achieve optimal patient outcomes Guide patients and their families in understanding therapeutic objectives, discharge protocols, and at-home exercises Ensure accurate documentation of evaluations, treatments, and progress as per facility and regulatory standards Engage in interdisciplinary meetings and care rounds to foster organized patient care Qualifications

Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution Licensure and Certifications

Valid state licensure as a

Physical Therapist Preferred CPR certification Specialized certifications (e.g., in neurology or geriatrics) are beneficial, though not mandatory Key Skills
