Description Assess patients in both inpatient and outpatient environments and develop personalized therapy plans tailored to individual needs Implement various physical therapy techniques with a focus on mobility, strength, balance, and overall functional recovery Work harmoniously with nurses, physicians, case managers, and other rehabilitation specialists to optimize patient outcomes Guide patients and their families in understanding therapeutic goals, discharge planning, and home exercise routines Accurately document patient evaluations, treatment progress, and care plans per available guidelines Participate in multidisciplinary team meetings and care rounds to ensure seamless coordination of care Education

Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution Certifications

State licensure as a

Physical Therapist CPR certification recommended Specialized certifications (e.g., neurological, geriatric specialties) are advantageous but not mandatory Skills

Prior experience in this field is highly desirable Ability to manage patients with complex medical and mobility challenges Excellent interpersonal and teamwork abilities Familiarity with digital charting and clinical documentation systems Benefits
