Description Conduct assessments for patients in either an inpatient or outpatient setting and craft tailored treatment plans Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and functionality Collaborate with nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes Educate patients and their families on therapeutic objectives, discharge plans, and home exercise routines Maintain records of evaluations, treatments, and progress according to facility and regulatory norms Engage in interdisciplinary conferences and care team rounds to ensure seamless patient care Education

Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution Certifications

State licensure as a

Physical Therapist CPR certification is preferred Desirable specialized certifications (e.g., neurological, geriatric) Skills

Prior experience is highly preferred Capability of addressing patients with intricate medical and mobility challenges Excellent communication and collaborative skills Familiarity with digital charting and clinical documentation systems Benefits
