Description Conduct patient evaluations in both inpatient and outpatient environments, crafting personalized treatment plans tailored to their needs Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function Work collaboratively with nurses, physicians, case managers, and a broad spectrum of rehabilitation professionals to maximize patient outcomes Educate patients and their families about therapeutic objectives, discharge processes, and at-home exercise routines Keep detailed records of patient evaluations, treatments, and progress that align with facility protocols and regulations Engage actively in care team discussions and interdisciplinary meetings to ensure holistic patient care Education
Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized graduate program Certifications
Active state licensure as a
Physical Therapist CPR certification is highly preferred Specialized certifications (e.g., geriatric, neurological) are valued but not mandatory Skills
Prior experience is strongly preferred Ability to care for patients with intricate mobility and medical conditions Excellent communication and collaborative abilities Comfortable navigating digital health records and clinical documentation systems Benefits
