Description Conduct evaluations of patients in either inpatient or outpatient settings and establish customized treatment plans Implement physical therapy interventions centered on improving mobility, strength, balance, and function Collaborate with nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes Educate patients and their families regarding therapeutic objectives, discharge planning, and home exercise regimens Maintain thorough documentation of evaluations, treatments, and progress in line with facility and regulatory guidelines Engage in care team rounds and interdisciplinary meetings to ensure seamless care coordination Education
Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution Certifications
Valid state licensure as a
Physical Therapist CPR certification preferred Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory Skills
Prior experience is highly recommended Ability to treat patients with complex medical and mobility challenges Excellent communication and teamwork abilities Proficient in using digital charting and clinical documentation systems Benefits
