Description Assess patients in either inpatient or outpatient environments and craft tailored treatment plans Deliver physical therapy services focused on enhancing mobility, strength, balance, and overall function Collaborate with nurses, physicians, case managers, and other rehabilitation professionals to achieve optimal results Guide patients and their families through therapeutic objectives, discharge strategies, and at-home exercise regimens Maintain detailed documentation of evaluations, treatments, and patient progress as per facility and regulatory standards Engage in care team rounds and interdisciplinary meetings to ensure seamless patient care Education
Doctorate in Physical Therapy (DPT) or equivalent qualifications from a recognized program Certifications
Active state licensure as a Physical Therapist CPR certification preferred Advanced certifications (e.g., neurological, geriatric) are advantageous but not mandatory Skills
Previous clinical experience highly preferred Capability to manage patients with complex medical and mobility challenges Exceptional communication and teamwork abilities Proficiency with electronic health records and clinical documentation systems Benefits
