Description Conduct thorough evaluations for patients in diverse settings, crafting personalized treatment plans tailored to their unique needs Implement effective physical therapy interventions that prioritize mobility, strength enhancement, balance, and overall function Engage collaboratively with a team of nurses, physicians, case managers, and rehabilitation professionals to achieve optimal patient outcomes Guide patients and their families through educational discussions on therapeutic objectives, discharge planning, and at-home exercise techniques Maintain meticulous documentation of evaluations, treatments, and patient progress, adhering to both facility and regulatory standards Contribute actively in care team rounds and interdisciplinary meetings to promote coordinated care Education
Doctorate in Physical Therapy (DPT) or an equivalent qualification from a recognized program Certifications
Must possess state licensure as a
Physical Therapist CPR certification is preferred Any specialized certifications (e.g., in neurological or geriatric therapy) are advantageous but not mandatory Skills
Prior experience is highly desirable Ability to effectively treat diverse patients with complicated medical and mobility challenges Strong communication skills and a collaborative spirit Proficiency in utilizing digital record-keeping and clinical documentation systems Benefits
