Miami-Dade Corrections Health Services (CHS) operates under Jackson Health System (JHS) and serves approximately 5,000 patients daily. Patient care is delivered by nearly 400 healthcare employees representing a diverse multidisciplinary team of nurses & nurse support staff, physicians, psychiatrists, social workers and other professionals. CHS supports Miami Dade Corrections & Rehabilitation at 3 facilities in Miami-Dade County and leverages the resources of a large public safety net health system in the areas of education, quality, risk management & compliance.

Summary

Clinical Practice Pharmacist performs a variety of pharmacy procedure requiring professional knowledge and skills in a large and complex department of pharmacy. Emphasis of the work is on compounding and dispensing a variety of medications, manufacturing of more complex solutions and in supervising work of pharmacists not registered within the state. All work is performed in accordance with recognized professional standards, hospital and pharmacy rules and regulations and applicable state and federal laws.

Responsibilities

Integrates drug distribution and clinical pharmacy services in the pharmaceutical care model in order to assure safety and efficacy, and to reduce costs. Evaluates physicians orders, prescriptions, vouchers, and cart fill lists for appropriateness of therapy, taking into account drug interactions, allergies, and relevant patient conditions. Assists in the collection of data for drug utilization evaluations. Assists in the collection of data for drug utilization evaluations. Provides drug therapy monitoring and intervention through target drug programs depending on area assignment. Serves as a role model and educator for pharmacy students performing internships and externships, and pharmacy residents at JHS as required. Participates in topic discussions with clinical clerkship students when requested. Provides in-services to nursing and/or other health care professionals. Actively participates in staff development programs, providing programs when requested. Provides therapeutic drug level monitoring (e.g. recognizes need for levels, schedules and interprets levels). Reports adverse drug reactions encountered by their assigned patients. Demonstrates behaviors of service excellence and CARE values (Compassion, Accountability, Respect and Expertise). Performs other related duties as required.

Experience

Generally requires 5 to 7 years of related experience.

Education

Pharm.D. degree or B. S. with equivalent clinical knowledge is required.

Credentials

Valid Florida pharmacy license or certification as required.
