Clinical Hospital Pharmacist by using established policies and procedures is responsible for: drug therapy of assigned patients; provision of clinical drug information to meet the needs of the physician, nurse, and patient; education of pharmacy students, residents, and interns; and distribution of medications. Duties & Responsibilities
MS: Evaluates medication therapy for appropriateness, considering patient conditions (age, allergies, renal status, comorbidities, etc.), safety and efficacy of the prescribed medication (identity, strength, dose, frequency, duration, directions), formulary status, potential drug–drug or drug–food interactions, and alternative therapies (cost or efficacy). Provides therapeutic drug level monitoring when appropriate. Communicates effectively with nurses and prescribers to correct errors or optimize therapy. Consults with other practitioners as appropriate (other Clinical Hospital Pharmacists, Pharmacy Management, Attending Physician, etc.). ES: Identifies trends during routine pharmaceutical care, collects data, and formulates practice recommendations to improve outcomes or reduce cost. Presents data to the appropriate committee as required. Maintains exceptional feedback from medical staff regarding the pharmacist’s impact on patient outcomes. Performs formal, written consults and documents all clinical consult information in the medical record per hospital guidelines. MS: Demonstrates accuracy when dispensing, preparing, labeling, or filling automated dispensing machines and with computer data entry. ES: Identifies ways to decrease the possibility of medication dispensing errors. MS: Completes tasks with appropriate speed without sacrificing accuracy. ES: Is efficient enough to accomplish additional tasks upon request. MS: Demonstrates reliability and a conscientious, thorough approach to work. Demonstrates initiative, adaptability, and willingness to adjust schedules to support patient care or departmental needs. MS/ES: Completes work left from other shifts and assists others as needed; proactively suggests improvements in procedures or systems to enhance service, patient outcomes, cost control, and asset protection. MS: Demonstrates effective time-management and prioritization; meets deadlines; communicates limitations or needs for clarification to supervisors. MS: Demonstrates communication and problem-solving skills; performs routine duties with minimal supervision; proposes solutions to concerns. MS: Develops the skills of others and may participate in staff orientation, education/training of pharmacy students and healthcare personnel. Supports the improvement of junior staff performance through coaching; ensures area-specific quantitative requirements are met and documented. MS/ES: Participates in teaching activities, presentations or publications; contributes to ongoing professional development and committee work. MS: Maintains proficiency with area-specific technology and techniques (e.g., automated dispensing, computer systems, aseptic techniques). ES: Proactively suggests enhancements to area technology and processes. MS: Identifies opportunities for DUE or Target Drug Programs (TDP) and contributes to related activities; presents results to the appropriate committee; ensures outcomes influence practice changes. Takes initiative to develop and implement new services or programs related to productivity, QA, or performance improvement. MS: Ensures documentation of adverse drug reactions, clinical interventions, incident reports, and medication errors; supervises supportive personnel and optimizes workflow. Assures compliance with routine tasks and assists in performance evaluation as requested. Demonstrates CARE values (Compassion, Accountability, Respect, and Expertise). Experience and Qualifications
Required Qualifications:
Generally requires 5 to 7 years of related experience. Education:
Pharm.D. degree and residency training or equivalent proven experience is required. Credentials:
