Description Conduct patient evaluations in either inpatient or outpatient contexts and develop personalized treatment plans Implement physical therapy interventions that enhance mobility, strength, balance, and function Work alongside nurses, physicians, case managers, and fellow rehabilitation specialists to ensure optimal outcomes Guide patients and their families through therapeutic goals, discharge processes, and home exercise programs Maintain thorough documentation of evaluations, therapies, and patient progress in compliance with facility and regulatory standards Engage in interdisciplinary care rounds and conferences to facilitate coordinated care Education
Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution Certifications
Active state licensure as a
Physical Therapist CPR certification preferred Specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory Skills
Preferably previous clinical experience in diverse settings Capable of managing patients with intricate medical and mobility challenges Excellent communication and collaborative skills Adept at utilizing digital charting and clinical documentation systems Benefits
