We are seeking a driven and experienced

Practice Transformation Specialist

to support the successful implementation of

value-based care

initiatives across our clinical network. This role is essential in guiding clinical teams through transformational change to improve outcomes, optimize workflows, and advance population health goals. The ideal candidate is passionate about community-based care, understands the unique needs of Federally Qualified Health Centers (FQHCs), and thrives in a collaborative, data-driven environment.

Responsibilities

Serve as a trusted advisor to health centers, fostering strong relationships and supporting value-based care transformation.

Lead initiatives that enhance clinical and financial performance through data analysis and workflow optimization.

Provide hands-on training and support on population health tools, care management strategies, and performance reporting.

Conduct in-depth analysis of clinical, operational, and financial data to inform continuous quality improvement.

Collaborate with cross-functional teams to drive alignment, engagement, and measurable outcomes.

Required Qualifications

Bachelor’s degree in Public Health, Healthcare Administration, Nursing, or a related field (Master’s preferred).

Minimum 3 years of experience in healthcare quality improvement, clinical operations, ACOs, or care transformation.

Experience working with FQHCs or in a value-based care environment.

Deep understanding of population health, HEDIS, and care coordination strategies.

Strong communication, facilitation, and relationship-building skills.

Proficiency with EHRs, Microsoft Office, and healthcare data reporting tools.

Preferred Skills

5+ years in provider relations, practice management, or healthcare project leadership.

Background in process improvement methodologies (e.g., LEAN, PDSA).

Familiarity with risk adjustment, commercial payer models, and MassHealth ACOs.

Strong commitment to health equity and mission-driven work.

Location This is a remote position; however, candidates must reside in South Florida (Miami-Dade or Broward County) to build a relationship with our partner.
