Adolescent Substance Use Disorder Counselors
to support our growing teams. The first Substance Use Disorder Counselor is a member of our SUD Team and will provide confidential counseling, education, and support to individuals and families experiencing substance use disorders, and for promoting health lifestyles and healthy choices to individuals, families, and the community. The other Substance Use Disorder Counselor is a member of our new CCBHC Team and will provide integrated dual diagnosis disorder treatment to clients who have substance use concerns. The SUD Counselor is responsible for providing confidential counseling, education, and support to individuals and families experiencing substance use disorders, and for promoting health lifestyles and healthy choices to individuals, families, and the community. Provide a range of prevention, assessment, and intervention services. Provide training and consultation to team members on problems, assessments, and recommendations for clients. Use IDDT treatment and Motivational Interviewing techniques. About The Centers & Circle Health Services

Must have experience in monitoring outcomes data and performance indicators. Must possess computer proficiency and proficiency working in an EMR. here . Essential Job Duties & Responsibilities
Provide individual, group counseling, crisis intervention and case management services. Provide counseling to pregnant women about the effects of drugs and alcohol on the fetus, as requested. Will utilize evidence based approaches to care, including motivational interviewing, trauma informed care and strength based therapy. Work collaboratively with behavioral health, physical health and pharmacy staff to ensure integrated care is available to all clients. Develop treatment plans with client and team that are complete, individualized and based on needs from assessment and those identified in the course of treatment. Document accurately and completely all care provided using appropriate records and within allotted time frame. Actively monitor outcomes and make real time adjustments to ensure program meets targets. Develop and facilitate communication and education plan to help increase addictions awareness among BH staff.
