The Department of Health Services (DHS) Southern Wisconsin Center (SWC) is recruiting for a Qualified Intellectual Disability Professional (QIDP). This position will typically work between 7:45am - 4:30pm, Monday through Friday. One alternate shift may be required periodically to meet the centers needs and may be flexible based on the supervisors approval. 3.5 weeks of vacation, 9 paid holidays and ample sick time. SWC is a state run, licensed and certified Intermediate Care Facility for Individuals with Intellectual and Developmental Disabilities (ICF/IDD). SWC provides a wide variety of programs and services designed to assist and prepare individuals to live successfully in the setting that best meets their needs. Position Summary As a Qualified Intellectual Disability Professional, you will be responsible for: coordinating the development of comprehensive functional assessments and Individual Program Plans and monitoring and integrating resident programs to ensure continuous active treatment. conducting special staff training and monitoring all aspects of the residents Individual Program Plan ensuring appropriate staff address areas of concern, and documenting and monitoring documentation of resident treatments. Please note, you must be a Qualified Intellectual Disability Professional (QIDP) as defined in the Code of Federal Regulations. Salary Information The starting salary is between $31.00 $36.00 per hour ($64,400 -$74,800 annually), depending on qualifications plus excellent benefits. A 12-month probationary period is required. Pay for current or former state employees will be set based on the rules that apply to compensation for the applicable transaction.This position is classified as a Developmental Disabilities Specialist is in pay schedule and range 11/04. All applicants who may be appointed to this position will be required to allow DHS to conduct a background check to determine whether the circumstances of any conviction may be related to the job being filled. All appointees will be required to receive a TB test, which will be administered by SWC or medical personnel approved by the facility prior to their start date. Thispositionhasphysical requirements which may include but are not limited to lifting/ carrying up to 55 lbs, pushing up to 150lbs to 200 lbs (e.g. wheelchairs, carts, etc.), bending, kneeling, stooping, standing, and walking on hard tile floors for extended periods of time. Applicants may be required to pass a physical evaluation to assess these requirements. DHS does not sponsor work visas for this position, thus, in compliance with federal law, selected candidates will be required to verify eligibility to work in the United States by completing the requiredI-9 form upon hire. Qualifications Minimally qualified applicants are required to meet the definition of Qualified Intellectual Disability Professional (QIDP) defined as: A professional who has at least one year of experience working directly with persons with intellectual disabilities or other developmental disabilities

AND Is one of the following: (1) a doctor of medicine or osteopathy; (2) a registered nurse; or (3) an individual who holds at least a bachelors degree in a professional category specified in the Federal Register as meeting the QIDP qualifications as outlined in 483.430 of the Code of Federal Regulations . Note: Official transcripts will be required prior to appointment to the position Well-qualified applicants will also have one or more of the following: Experience working with individuals with intellectual disabilities dually diagnosed with a mental health condition. Experience developing comprehensive functional assessments and Individual Program Plans. Experience monitoring and integrating resident programs to ensure continuous active treatment.
