Overview

Qualified Intellectual Disability Professional II at Texas Health and Human Services. Location: CARLSBAD, TX. The state-wide Centers serve individuals with intellectual and developmental disabilities who are medically fragile or have behavioral challenges, providing campus-based direct services and supports at 13 centers. The QIDP II is central to the active treatment program, overseeing the ISP process, coordinating an interdisciplinary team (IDT), ensuring assessments, and facilitating services and supports to individuals on the caseload. This role requires strong organizational skills, the ability to balance priorities, flexible scheduling, and proficiency with electronic records systems. Responsibilities

Orchestrate and oversee the ISP process for all individuals on the assigned caseload, including ISP Preparation, Addendum, and Annual ISP meetings. Ensure assessments and evaluations are completed to determine functioning, preferences, strengths, needs, and recommendations. Coordinate and facilitate participation of the individual and, if appropriate, the Legally Authorized Representative (LAR) with staff in developing and delivering needed supports and services. Ensure timely follow-up on recommendations for supports, services, equipment, or programs. Observe and interact with individuals across settings; collect and analyze data on ISP implementation and effectiveness. Train/coach staff in service implementation and behavior management; provide leadership and role-model behavior for staff at multiple levels. Collaborate with behavioral health staff to integrate behavioral needs into the ISP. Monitor and evaluate ISP implementation monthly; document analyses and recommend interventions or revisions as needed. Ensure client protections are in place and follow Human Rights Committee processes for potentially restrictive practices; advocate for individuals. Maintain documentation in electronic records in compliance with ICF/IID standards and deadlines. Support agency disaster response/COOP readiness and may require alternate shift patterns. Knowledge, Skills and Abilities (KSAs)

Knowledge of the IDT process and people-centered planning. Knowledge of rehabilitation and skills acquisition programs for persons with intellectual disabilities; ability to design programs and analyze data. Knowledge of PBSPs and PNMPs; ability to coordinate clinical services and supports. Ability to prepare and maintain records; strong verbal and written communication; problem-solving; independent work and prioritization. Ability to adapt quickly to change; ability to work professionally with staff and LARs. Ability to perform direct care as needed; facilitate IDT meetings and use PC software and electronic records systems. Qualifications

Bachelors degree from an accredited college or university in psychology, sociology, social work, special education, rehabilitation counseling, or related human services field (or equivalent subject matter with transcript review). At least one year of experience working or volunteering directly with persons with intellectual or developmental disabilities. Additional Information

Applicants must pass fingerprint background checks, pre-employment drug screen, and registry checks (CANRS, Nurse Aide, Medication Aide, and LEIE). Males 1825 must be registered with Selective Service; random drug testing applies; flexibility to work weekends/holidays; may require overtime.
